RESUMO

Despite historic existence of campus ministries at universities, little is known about the roles of campus ministry leaders. This research explored campus ministry leaders' engagement with students through interviews (n = 19). Analysis indicated interviewees' work includes building relationships with students, navigating the secular context of a public university, and tensions with others in campus ministry. Interviewees reported emotional strain resulting from extensive caring labor. Focused support for campus ministers would benefit both leaders and students.

RESUMO

Guided by the Anderson Behavioral Model, the researchers in this study examined the association between emotional violence and HIV testing uptake among women, using the 2016 Ethiopian Demographic and Health Survey. Only 3,637 women between the ages of 15 and 49 years were included. In the bivariate analysis, emotional violence, reported by 42.6% of respondents, was associated with prior HIV testing. In the multiple logistic regression model, emotional violence was not associated with HIV testing uptake among women (p > 0.05). Our findings suggest that efforts to improve women's socio-economic status and decision-making autonomy may be more relevant to increasing HIV testing uptake.

RESUMO

HIV testing is a cornerstone for early HIV diagnoses which can improve quality of life, survival, and reduce forward transmission. This study examined socioeconomic determinants of HIV testing among women in Ethiopia using the 2016 Ethiopian Demographic and Health Survey. The sample was collected using stratified cluster sampling design and was selected in two stages. A total of 15,683 women aged 15-49 completed the survey. For this study, only 8681 participants were included. Kabeer's theoretical framework on women's empowerment was used for variable selection and analysis. Chi-square test and multiple logistic regression modeling were performed. Overall, 52% of the women reported testing for HIV. In the multivariable-adjusted model, education, residence, wealth index, occupation, living arrangement, and healthcare decision making were significantly (P < 0.05) associated with ever being tested for HIV. For instance, women who don't participate in the decision making of their own health care were less likely to have ever been tested (AOR: 0.77 (0.63-0.94) than those who do. This implies that HIV prevention among Ethiopian women presupposes national policies that promote their overall socioeconomic empowerment. Providing more resources to women, especially to those living in rural areas, might increase HIV testing.

RESUMO

Peer teaching provides students with opportunities to experience the educator role and increase self-confidence and problem-solving skills. To address the shortage of meaningful leadership experiences for senior baccalaureate nursing students, faculty implemented an alternative leadership experience involving senior students taking on the role of peer teachers in the high-fidelity simulation (HFS) laboratory. We conducted focus groups to assess peer teachers and learners' experiences and used thematic analysis to examine and interpret the data. Findings included the intersection of learning, HFS, clinical experience, and teaching; the benefits of practicing teaching in a safe environment; how empathy enhanced understanding of the continuum of expertise; and power differentials experienced by peer teachers and learners.

RESUMO

PURPOSE: Language asymmetry between patients and providers may influence the context, content, and quality of health care communication, affecting patient outcomes and contributing to health disparities. This research examined interpreter-mediated, primary care encounters between English-speaking nurse practitioners and Spanish-speaking adult patients. METHOD: Situational analysis guided the collection, analysis, and interpretation of audio-recorded clinical encounter data. RESULTS: Interpreter-mediated communication was situated within intersecting social, economic, political, and health systems contexts. Three modes of collaborative knowledge generations were Constructing Connections, Constructing Mutual Understanding, and Constructing Effective Systems Navigation Strategies. DISCUSSION: These findings illustrate how interactants contributed individual and collective knowledge across multiple systems to address patient concerns. CONCLUSION: The analysis revealed ways in which communication processes may influence both providers' diagnostic and interventional decision-making and patients' understanding and potential compliance. Ongoing preparation and support for intraprofessional collaboration is needed to ensure effective communication and mitigate untoward effects of language asymmetries in clinical encounters.

RESUMO

PURPOSE: Tummy Time is preventive positioning designed to strengthen an infant's neck and upper body muscles and minimize musculoskeletal disorders, including positional plagiocephaly. Latino parents with limited English proficiency (LEP) may experience barriers to receiving instruction on this important health-promotion strategy. Study aims included 1) adaptation and implementation of a Spanish-language version of Tummy Time; 2) evaluation of participant Tummy Time knowledge; and 3) assessment of efficacy and acceptability of the Spanish language intervention. DESIGN AND METHODS: This feasibility study involved translation of educational materials into Spanish, creation of Spanish-language print materials, and training of bilingual research assistants to deliver the educational intervention. Participants were 21 Latino expectant and current parents with self-identified LEP. Participants completed brief pre- and post-tests and performed a return demonstration. RESULTS: Participant knowledge of Tummy Time increased post-intervention. Program evaluation and on-site observations supported feasibility and acceptability. CONCLUSIONS: The cultural and linguistic tailoring of targeted educational interventions is a feasible and potentially effective strategy to overcome barriers experienced by parents with limited English proficiency and may contribute to improved infant health outcomes. PRACTICE IMPLICATIONS: To be effective, health education programs must be tailored to the target population. Pediatric nurses and nurse practitioners should be aware that populations with LEP face many barriers that may inhibit their ability to participate in and understand health promotion educational activities. While translating interventions can mitigate language barriers, providers should also evaluate and address other barriers such as time constraints, transportation issues, and trust.

RESUMO

Background: Language, culture, geographic, social, and economic factors are associated with health disparities. Among more recent Hispanic immigrants, limited English proficiency and immigration status are barriers to health information and healthcare access. Improved access to culturally and linguistically tailored health information through technology could potentially enhance healthcare access and health outcomes. However, little is known about health information-seeking through technology among Hispanics in recent settlement areas. Purpose: The aim of this exploratory study was to describe patterns of self-reported utilization of technology for health information-seeking among the growing Hispanic population in South Carolina (SC) over a period of 5 years. Methods: Descriptive, community-based, cross-sectional survey of 361 Hispanic adults residing in SC, conducted in 2011 and 2015/2016. Results: Reflective of reported national trends, self-reported accessibility and utilization of cellphones increased (89-96.6%) among this sample. Although computer ownership decreased (58-53.9%), internet utilization for health information-seeking increased (45-57.8%); more than 80% of participants indicated that they considered the internet a "good source of health information." The majority of participants in both time periods conducted health information searches in Spanish, although the reported access to English-language information increased over time. Conclusions: These findings illustrate the increasing access and utilization of technology for health information among Hispanics in SC, underscoring the need for broader dissemination of culturally and linguistically appropriate health information through accessible technology, including Spanish language websites. Recommendations for future research include examining relationships between technology access, health service access and utilization, and health behaviors among Hispanics in diverse geographic and social contexts.

RESUMO

In this descriptive qualitative research, we examined older women's responses and decisions after experiencing a fall. Falls were unexpected, sudden events that heightened these women's awareness of their physical, emotional, spiritual, and social independence. Interviewees reported assessing personal, physical, and emotional needs; feeling burdened by the extra work; trying to get back to normal; seeking and obtaining assistance and spiritual support; avoiding specific people, objects, and places; planning ahead; and putting the fall out of mind. Consideration of older women's post-fall responses and decisions should be incorporated into fall prevention and management programs, services, and clinical recommendations.

RESUMO

With the aim of addressing inequalities and disparities in health care access and outcomes, in 2001 the United States Department of Health and Human Services Office of Minority Health (OMH) established National Standards for Culturally and Linguistically Appropriate Services (CLAS). In 2010 the OMH solicited public, private and government input which was incorporated into the Enhanced National CLAS Standards. To date there have been no formal reviews of the published literature on the CLAS Standards. The aim of this scoping review was to identify the scientific and professional literature related to the CLAS standards and describe the content, focus, conceptualization and application of these publications, with the goal of providing insights and directions for further research and application of the CLAS standards.

RESUMO

The use of promotoras de salud is an increasingly widespread delivery approach for community-based health education and promotion programs targeting obesity-related lifestyle behaviors for Hispanic populations. Addressing a gap in the literature, this research examined the sustainability of promotora-led initiatives from the perspectives of those who plan, implement, and evaluate these programs. We conducted 24 in-depth interviews with program planners representing 22 promotora programs focused on Hispanic women's health in 10 states. Findings illustrated program planners' opinions regarding the components, logistics, and barriers to promotora program sustainability. Several participants challenged the notion of promotora program sustainability by reframing the issue as promoting individual promotoras' well-being and social mobility rather than maintaining their role in the program over time. Implications for community health planning, management, and policy include developing sustainability strategies during program planning stages and implementation of policies to more effectively integrate promotoras into existing health care systems at local, state, and national levels.

RESUMO

There is widespread recognition of the cultural and linguistic appropriateness of (a Spanish term for community health workers [CHW]) in health education and outreach among Hispanic communities. Yet, there are significant gaps in the literature regarding the preparation, implementation, and evaluation of ' engagement in research. To address this gap, we examine ' research-related training, roles, responsibilities, and contributions in a community-based participatory research project involving a multisite randomized controlled trial (RCT) of a physical activity intervention for Mexican-origin women in Texas and South Carolina. We identify both benefits and challenges associated with ' engagement as community researchers; examine variations and differences in roles and responsibilities related to the research contexts, sites, settings, and individual characteristics; and discuss implications for research and practice.

RESUMO

This research sought to elicit HIV/AIDS peer counselors' perspectives about delivery formats for a counseling intervention. Peer counselors identified personal contact as the major advantage of the face-to-face format. Personal contact afforded counselors better opportunities to understand and assess clients' physical, emotional, and environmental status and allowed them to connect with peers in more concrete and personal ways. Being physically present was also a very direct and effective way to role model for other HIV-positive women. Peer counselors identified a number of inherent barriers and challenges to telephone interventions but also recognized potential logistic and personal advantages. Despite the overwhelming preference for the face-to-face intervention format, counselors acknowledged the potential for conducting successful peer counseling over the telephone. A significant finding was that the value and meaning of HIV/AIDS peer counselors' work transcended the limitations of either delivery format.

RESUMO

Current literature on community-based youth empowerment programs provides few specific operational descriptions of adult roles. This research addressed that gap by exploring the perspectives and experiences of adults actively engaged with youth empowerment programs. Data were gathered through in-depth interviews, field observations, and interactive group discussions with adult program leaders. The following dimensions of adults' work were identified: putting youth first; raising the bar for youth performance; creating the space and making things happen; being in relationships; exerting influence, control, and authority; and communicating and connecting with the broader community. These findings provide guidance for the development of best practices in community-based youth empowerment programs.

RESUMO

BACKGROUND: The relationships between poverty, empowerment, and health are of theoretical and practical interest to nurses, yet few researchers have examined volunteerism in poor communities from a holistic health perspective. PURPOSE: This study explores the experiences of women engaged in community volunteer work in the context of economic poverty. METHOD: Individual, in-depth interviews were conducted with 8 women, ages 21 to 77 years, who qualified for federal assistance or self-identified as low-income and were currently involved in volunteer community work. FINDINGS: Participants' stories of being involved and making a difference illustrated women making connections, developing relationships, gaining knowledge and skills, increasing self-esteem and confidence, reaching out to help others, and strengthening communities. CONCLUSION: At both the individual and community level, well-being was enhanced through women's community volunteerism in the context of economic poverty. IMPLICATIONS: At the individual and community levels, well-being was enhanced through women's community volunteerism in the context of economic poverty. From a holistic nursing perspective, the findings provide support for community volunteerism as a health-promoting strategy among women living in economic poverty.

RESUMO

The purpose of this integrated review was to examine research on the relationships between immigration and health. The review was limited to studies of immigration into North America published since 1994. The results suggested that, although recent research has furthered the understanding of immigration and health, the multiple health effects of the various social and cultural processes immigrants undergo are still not clearly understood. In addition, research on acculturation has not clarified the positive or negative effects of acculturation on health. The incorporation of transnational perspectives and contemporary concepts and frameworks such as biculturalism, undocumentedness, and transitions was noted as a significant contribution from recent research. The results of this integrative review indicate that interdisciplinary research on immigration and health is moving in new directions. The reviewers provide suggestions for future research on health disparities as well as on possible health protective factors among diverse immigrant populations.
